What is Prostatitis?
Prostatitis is an inflammation of the prostate, and is one of the most common reasons men visit the doctor in the western world. It is most common in men over the age of 30, and particularly in men over the age of 60. While prostatitis is treatable, diagnosis can be lengthy and not all treatments are successful. This is partly because the various causes of prostatitis are not fully understood.

There are three main types of prostatitis:

  1. Non-bacterial prostatitis, which may develop suddenly or follow a slower or variable course. It is now more commonly called chronic male pelvic pain syndrome because it cannot be proved to be "non-bacterial", though a bacterial cause cannot be identified.

  2. Acute prostatitis, which develops suddenly and may not be permanent

  3. Chronic (bacterial) prostatitis, which develops gradually and may only have subtle symptoms, though it often continues for a prolonged period.

  4. Asymptomatic inflammatory prostatitis – which has no symptoms at all but results in an inflamed prostate, is sometimes mentioned. It has been discovered when biopsies are conducted on the prostate to rule out cancer, and no cancer is found. This is an histological and not a clinical diagnosis. Prostatitis is often reported on the histological analysis of TURP specimens when the prostate resection was performed for symptoms of BPH. If the patient is asymptomatic this histological finding does not warrant any treatment.


What Are the Symptoms?
Symptoms of long-term (chronic) prostatitis are often mild and start slowly over weeks or months. They may include:

  • A burning pain when you urinate.
  • A problem starting the urine stream, urinating in waves rather than in a steady stream, urine flow that is weaker than normal, and dribbling after urinating.
  • An urge to urinate often. But you may pass only small amounts of urine.
  • Waking up at night to urinate often.
  • A feeling of not completely emptying your bladder.
  • Pain in your lower back, in the area between the testicles and anus, in the lower belly or upper thighs, or above the pubic area. Pain may be worse during a bowel movement.
  • Some pain during or after ejaculation.
  • Pain in the tip of your penis.

Symptoms of acute prostatitis are the same, but they start suddenly and are severe. They may also include a fever and chills.

Some men may have no symptoms.

What Causes Prostatitis?
In some cases, prostatitis is caused by bacterial infection. These cases are treated with antibiotics to kill off the infection. But the more common forms of prostatitis are not associated with any known infecting organism. Antibiotics are, therefore, often ineffective in treating these forms of prostatitis.

The four main types of prostatitis and their causes are described in the following sections.

How is it Diagnosed?
Your healthcare provider will ask about your symptoms. Your provider will examine your abdomen and scrotum to check for other possible problems. Tests you may have are:

  • tests of a sample of urine
  • a rectal exam
  • tests of fluid from the prostate.

Your healthcare provider may ask you to give a urine sample before a rectal exam of the prostate. During the rectal exam, your provider will press on the prostate. This will cause fluid from the prostate to enter your urinary tract. After the rectal exam you will provide another sample of urine. This will help your provider know if the prostate or the urinary tract is infected.

Your provider may conclude that you have nonbacterial prostatitis if you have symptoms but neither the urine culture nor the prostate fluid show evidence of infection.


Treatment by Western Medicine
Prostatitis treatments vary depending on the underlying cause. They can include:

  1. Antibiotics. This is the most commonly prescribed treatment for prostatitis. Your doctor will base the choice of medication on the type of bacteria that may be causing your infection. If you have severe symptoms, you may need intravenous (IV) antibiotics. You'll likely need to take oral antibiotics for four to six weeks, but may need longer treatment for chronic or recurring prostatitis. Take all of the prescribed drugs as directed even if you're feeling better. Otherwise, treatment may not work. Your doctor may have you try one or more antibiotics even if the cause of your prostatitis can't be identified. If antibiotics don't help, your prostatitis is most likely caused by something other than a bacterial infection.
  2. Pain relievers. Pain medications such as aspirin or ibuprofen (Advil, Motrin, others) may make you more comfortable. You should discuss with your doctor what doses you can safely take. Overusing these medications can cause problems.
  3. Alpha blockers. These medications help relax the bladder neck and the muscle fibers where your prostate joins your bladder. This treatment may lessen symptoms, such as painful urination. Examples include tamsulosin (Flomax), terazosin (Hytrin), alfuzosin (Uroxatral) and doxazosin (Cardura). Common side effects include headaches and a decrease in blood pressure.
  4. Other treatments. Other potential treatments for prostatitis are being studied. These treatments include heat therapy with a microwave device and drugs based on certain plant extracts.

Alternative Medicine
Alternative therapies that show some promise for reducing symptoms of prostatitis include the following:

  • Biofeedback. This is a method for teaching you to use your thoughts to control your body. A biofeedback specialist uses signals from monitoring equipment to teach you to control certain body functions and responses, including relaxing your muscles. Some small studies have suggested the benefit of this process to manage pain associated with prostatitis.
  • Acupuncture. This type of treatment involves the insertion of very thin needles through your skin, to various depths at certain points on your body. A few small studies have shown that acupuncture may help with prostatitis symptoms.
  • Herbal remedies and supplements. There's no evidence that herbs and supplements improve prostatitis, although many men take them. Some herbal treatments for prostatitis include cernilton (rye grass), quercetin (a chemical found in green tea, onions and other plants) and extract of the saw palmetto plant. Prostate supplements combine minerals and vitamins, particularly zinc, selenium and vitamins E and D. Depending on the formulation, some supplements may be harmful, so talk to your doctor before using them.

Dosage:
Use ceramic pot, put in 1,000 - 1,200 ml water inside, then put all herbs into the water to soak for 30 minutes. Use soft fire to cook the water till it become boiled for 10 - 20 minutes and use soft fire to cook till around 200 ml is left. Pour out all water and cook the docoction second time, get the water as before. Mix up the medicinal water you get in two times. Then needle tubing the liquid into rectum two times everyday.

Precaution:
No alcohol, no wine, no spicy food.

Clinical experiment results:
82 patients received the treatment, 43 gained fully recovery, 21 improvement, 7 no results.
